It depends on your hardware. If your hardware doesn't support 320x240 then you can't, if it does then you can.
Most moden hardware won't support fullscreen resolutions below 640x480.

I just tried Darkplaces and Quakespasm on Linux, nvidia binary drivers for a 8800GTX with "Full Force GPU Scaling" enabled in the driver and "GPU Scaling Method" on "Aspect Ratio Scaled". Both worked like a charm.

"Aspect Ratio Scaled" sounds as though it's really a higher resolution, is rendering to a 320x240 texture then copying that up to the full screen size.

320*200 is where its at. highest framerates that way.

Id love to see a blood remake like any other old fan but the problem is the codes lost (confirmed by monolith)
a few have tried to recreate it using the jfduke source and the leaked alpha of the blood source for the missing parts,
but it newer made it out of very early alpha state and it was newer playable :/
Theres also some work done on a total conversion to eduke32 but its far from done.
Then theres transfusion which uses darkplaces but focussed on the online part so no singleplayer.
Id put my money on the total conversion but lets see.
