What is a good program to learn to make skins from scratch?

That depends on many things:

The model format you want to skin
If you want to draw on an image or directly on the model
The format you want to use for the image

if you want to draw an image, well, then the answer is "your favorite image editor". I recommend GIMP, it is free and fun.

All user preference. Hell I've seen some people damn good with MSPaint - but I'd NEVER recommend that lol

I use PaintShop Pro 7 - the later versions I didn't like (but have installed as they do have some features I use time to time). However 7 had the best interface.

I use Blender's internal painting tool.

OH SHIT, AN ALIEN !!! I DON'T USE PHOTOSHOP!
