OK, not fully thought out, but here goes:
* Team based deathmatch.
* Customizable class system with choices of weapons, and two abilities or class bonuses. Upon character completion, each character is saved, to be "logged into again"
* Each team has a Cthon or Shub-Niggurath that the opposing team must kill to win.
* Frags can be used to upgrade your character. Purely a currency. Maybe even an experience system.
* If experience is used, you get stronger as you gain levels.
Anyone have anymore ideas... I'm just sitting here thinking.

I remember Q3:TA having some game mode like this, where you had to destroy some enemy building to win.
But replace the regular player with Quake monsters... And then you're into something really interesting. A TF-style game using monsters instead classes:
shambler == heavy
ogre == demoman
scragg == spy (?)
vore == medic (?)
? == ?

I know FrikaC made a cgi-bin version of the quakec interpreter once and wrote part of his website in QuakeC (LordHavoc)
