Both. A lot of simple things can be improved, like better hitboxes, a more updated mapping format, and vastly improved graphics. However, I would expect them to expand upon (or provide a nearly completely different set of) weapons, monsters, etc. It's good to have familiarity, but not everything ...

I wonder what mechanics would be appropriate to include in a true Quake sequel. My concern is it's so easy and tempting to add all kinds of new features and ideas, but each thing you add risks taking you farther away from the original formula that worked so well. That's not the way to do it, imo ...
