I use the floating fading model idea in My Squad AI Thing, albeit in reverse, in order to make the screen fade to black on death.

q1bsp isn't actually an issue for anything other than crouching through small gaps, so it doesn't really matter at all.

I'll look for some more maps that might work for it.

Yeah, they all pretty much try to go to different firepoints, which makes them deadly on a map with so many routes through it.

No idea about the rocket fire; impulse 9 is not intended to be used, obviously. Probably a side effect from the grenade effects then.

If you're using darkplaces it can be done reasonably easily with a cl_prydoncursor, but if you want it to work in stock quake it's a lot harder. It can still be done, but it's not easy.

Funny bug if you play it in Engoo - the soldiers halt when they fire. then unfreeze when you distance from them. it's like playing statue sort of, at least in 1.1. It should support the sufficient extensions necessary for Squad AI including Tracebox so...

haven't tried (and should) 1.3 yet, can't ...
